Job description
Signify Health is seeking a Per Diem Clinician (NP or PA) to perform in-home health evaluations for members, delivering flexible, patient‑centered care.
Visits last about 45 minutes, including history review, physical exam, and any plan‑of‑care items; you may also conduct virtual visits and travel locally within licensed states.
Hourly base rate with additional incentives per visit; malpractice coverage is provided; supplies and other perks are included.
