Job description
Signify Health, part of CVS Health, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations, engaging with members in the comfort of their own homes and helping bridge gaps in care.
The visit lasts about 45 minutes and includes medication and medical history review, physical evaluation, and optional diagnostic tests per health plan. Travel within licensed states is required, with virtual visits as needed.
