Job description
Signify Health is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. You will help bridge care gaps by conducting health assessments in the comfort of patients' homes.
The role is flexible, allowing you to engage with patients one-on-one, review their medical histories, and utilize an intuitive clinical workflow. Ideal candidates will possess an active license and board certification, with opportunities to travel locally as needed.
