Job description
Signify Health, part of CVS Health®, seeks a part-time Clinician (NP or PA) to perform in-home health evaluations, visiting members at their homes.
You will conduct 45-minute visits, review medications and history, perform a physical evaluation, and coordinate with health plans. Travel locally within licensed states; virtual visits as needed.
