Monday-Friday, 3:00 PM-11:00 PM
Position Summary
The Licensed Practical Nurse (LPN) provides comprehensive nursing care and oversight for individuals receiving services within SRVS Intermediate Care Facilities (ICF). This role is responsible for coordinating and addressing all health-related needs of program participants through direct nursing care, health monitoring, medication management, and collaboration with interdisciplinary teams, families, and community healthcare providers.
The LPN plays a vital role in promoting the health, safety, and well-being of individuals served while ensuring compliance with regulatory requirements, medical protocols, and organizational standards.
Essential Duties and Responsibilities
Provide direct nursing care and health-related support to individuals served.
Administer medications and treatments as prescribed by physicians and authorized healthcare providers.
Monitor, assess, and document changes in individuals' health conditions and communicate concerns promptly.
Coordinate medical appointments, treatments, and follow-up care.
Maintain accurate and timely medical records in accordance with regulatory and organizational requirements.
Review, update, and implement health care plans as needed.
Collaborate with physicians, specialists, pharmacies, therapists, families, and interdisciplinary team members.
Respond appropriately to medical emergencies and health-related incidents.
Provide health education and training to direct support staff regarding medical protocols and individual health needs.
Monitor medication storage, inventory, and administration practices to ensure compliance and safety.
Participate in quality assurance activities, audits, and regulatory reviews.
Support infection prevention and control procedures within the facility.
Ensure a safe, healthy, and supportive environment for all individuals served.
Perform other duties as assigned.
Qualifications
Education and Experience
Graduate of an accredited Practical Nursing program.
Current Licensed Practical Nurse (LPN) license in good standing.
Minimum one year of nursing experience preferred.
Experience working with individuals with intellectual and developmental disabilities (IDD) or in long-term care, residential, behavioral health, or healthcare settings preferred.
Knowledge, Skills, and Abilities
Strong clinical assessment and medication administration skills.
Knowledge of nursing standards, healthcare regulations, and documentation requirements.
Ability to work independently while collaborating effectively with interdisciplinary teams.
Strong communication, organizational, and problem-solving skills.
Ability to maintain confidentiality and professional boundaries.
Proficiency with electronic health records and documentation systems preferred.
Physical Requirements
Ability to stand, walk, bend, reach, lift, and move throughout the workday.
Ability to lift and carry up to 50 pounds with or without reasonable accommodation.
Ability to provide direct assistance during medical emergencies and participant care activities.
Ability to work assigned schedules, including evenings, weekends, and holidays as required.
