Description
Licensed Practical Nurse
Join a Team That Makes a Difference! Trinity Health IHA Medical Group is seeking a compassionate and motivated Licensed Practical Nurse to support our providers in delivering exceptional patient care. In this role, you will prepare patients for a provider visit, provide results management support, insurance company advocacy, assist with clinical procedures, support care coordination and gaps in care and medication/vaccine administration in order to help create a positive healthcare experience every day.
What You'll Do
Welcome and prepare patients for appointments.
Determines and documents reason for the visit.
Uses critical thinking skills within the scope of the role to assess if patient information requires higher level follow up.
Collect and document patient histories, vital signs, and health information.
Assist providers with exams, procedures, testing, and specimen collection.
Administer medications and perform diagnostic testing as directed.
Communicate test results and provider instructions to patients.
Performs general specimen collection duties.
Provide patient and family education to support better health outcomes.
Coordinate care and serve as a liaison between patients and providers.
Support quality improvement initiatives and disease management programs.
General Requirements
EDUCATION: High School Diploma or GED and graduate of an accredited Licensed Practical Nurse Program. CREDENTIALS/LICENSURE: Valid, unrestricted LPN license in State of Michigan. Valid CPR certification. MINIMUM EXPERIENCE: 1-2 years of clinical experience in a medical or physician office. Previous nursing experience not required.
