Job description

Founded in 2017, Upperline Health is the nation's largest multispecialty physician group dedicated to transforming value-based care (VBC). We are redefining how care is delivered by integrating specialty care and primary care into a patient-centered and fully coordinated VBC model - enabling earlier intervention, improving outcomes, and enhancing quality of life for our patients.

By breaking down traditional silos between specialists, primary care providers, payors, and patients, Upperline delivers a more connected and holistic care experience through our VBC program, Upperline Plus . Our providers coordinate patients’ care and collaborate across a multidisciplinary team - including physicians, advanced practice providers, care navigators, pharmacists, dieticians, and social workers - to address both immediate and long-term patient needs.

Upperlineserves patients nationwide, in clinics and at home, delivering expert care in chronic condition management, podiatry, vascular, endocrinology, wound care, primary care, and more.

As we continue to expand, we remain committed to making high-quality, coordinated care more accessible and effective—while creating opportunities for our team to make a meaningful impact on the lives of the patients we serve.
