nAbout the Role:
nMedical Providers Partners PLLC is looking for a dedicated and compassionate Nurse Practitioner. This role is well-suited for early- to mid-career clinicians looking for meaningful work and a collaborative clinical environment.
nYou will provide comprehensive medical care to approximately 20 patients per day. This position offers autonomy, consistency, and the opportunity to build strong clinical relationships.
nResponsibilities – Skilled Nursing Facility Provider
nProvide assessment, diagnosis, and management of acute and chronic conditions
nConduct regular patient rounding and timely evaluations
nOrder and interpret labs, diagnostics, and imaging
nPrescribe and manage medications in compliance with regulations
nMaintain accurate, timely documentation in the EMR
nCollaborate with nursing, rehab, pharmacy, hospice, and administrative teams
nRespond to change-in-condition and urgent care needs
nSupport advance care planning and end-of-life discussions
nParticipate in quality assurance and regulatory readiness efforts
nQualifications
nActive Pennsylvania Nurse Practitioner license and required certifications
nGraduation from an accredited Nurse Practitioner program
nStrong attention to detail and timely documentation practices
nSelf-motivated, dependable, and patient-centered
nWillingness to participate in a rotating on-call schedule
