Job description
Pathways Healthcare in Worcester, MA is seeking a dedicated Nurse Practitioner to join our physician- and NP-led home health and hospice team. You will conduct admissions, create treatment plans, and provide routine and urgent home visits.
You will need an APRN license with DEA and CPR, 2+ years in home health, and strong communication. The role includes wound care, medication education, discharge planning, and care coordination. Mileage reimbursement and competitive per-visit rates are offered.
