Your Role: Key Responsibilities
Executing thorough patient assessments, directing diagnostic tests, and crafting personal care plans
Diagnosing and managing acute, chronic, or episodic conditions in collaboration with physicians or autonomously (as per credential guidelines)
Educating patients and families about health promotion, prevention strategies, and disease management
Prescribing medications and therapies in accordance with your practice scope
Engaging in patient follow-up and coordinating chronic care management
Working collaboratively with multidisciplinary teams to ensure seamless, holistic patient care
Meticulously documenting within electronic medical records (EMR)
Educational Background
Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
Graduation from a recognized Nurse Practitioner training program
Certain Certifications Required
Active Nurse Practitioner licensure in California
National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or specialty equivalent
Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications or the ability to secure them within 90 days
