Job description
ComplexCare Solutions seeks a Nurse Practitioner to provide per diem home and telehealth assessments nationwide. The NP will spend up to an hour with members to capture detailed personal needs and what is required to keep them at home and out of the hospital.
Eligibility includes an active NP license, ANCC/AANP board certification, and proficiency with EHRs. In-home visit rates vary by state; telehealth completions offer $85 per assessment. 401K eligibility is provided.
