nAs a Home Health Physical Therapist Assistant at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility and independence, from the comfort of their homes. By delivering personalized care that focuses on functional improvement, you'll empower individuals to overcome limitations and enjoy a better quality of life.
nPatient caseload located in South Kane & Dekalb County
nAs a Home Health Physical Therapist Assistant , you will:
nnnAdminister the physical therapy treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
n
nnAdminister non-complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities to patients.
n
nnDocument client records and submit clinical documentation to the office on time each week.
n
nnAttend educational meetings and coordinate department in-services.
n
nnNotify Physical Therapist or Branch Director of any equipment failures, lack of supplies, or staff or client changes.
n
nnApply concepts of infection control and universal precautions in performing patient care activities.
n
n nUse your skills to make an impact
nRequired Qualifications:
nnnCurrent state license to practice Physical Therapy
n
nnValid driver's license, auto insurance and reliable transportation
n
nnCurrent CPR certification
n
nnExperience collaborating with a team of healthcare professionals
n
nnComprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population
n
nnRead, write, and speak fluent English
n
n nPreferred Qualifications:
nnOne year of experience as a Physical Therapy Assistant
n nPay Per Visit/Unit Rate
n$41.00 - $57.00

nTB Statement:
nThis role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
nDriving Statement:
nThis role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.

nThe compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
