Take Your Therapist Home
As a PTA in this role, you will become an integral teammate in our innovative Take Your Therapist Home Program!
This role is focused on Home Health Care around our St. Clair Shores facility, with a steady referral source from multiple skilled nursing facilities.

In a world of shorter SNF stays, Take Your Therapist Home (TYTH) is a unique program where clinicians work directly with patients in both the SNF and home health settings, providing continuity of care and allowing you to have a greater impact on patient outcomes.
Rivetus has been serving the geriatric post-acute community since 1998. We are a specialized rehabilitation company operated and managed at the executive level by licensed clinicians, providing continuity of care across the Skilled Nursing, Home Health, and Outpatient settings.

Facility Location: Shorepointe - 26001 Jefferson Ave, St Clair Shores, MI 48081
This is a hybrid role providing Home Health therapy services in conjunction with SNF-based therapy services. The Home Health geographic area is in the vicinity of our Shorepointe facility. Home Health services are paid per visit, while SNF services are paid at an hourly rate.
Schedule: Monday - Friday, Full Time (Schedule flexibility Available!)
Required Qualifications: Successful completion of an accredited Physical Therapy Assistant program.
Licensure as required by the State of Michigan.
Valid Driver's License.
Skills and Abilities: Ability to exercise professional judgment and initiative when analyzing problems and recommending solutions.
Initiative and flexibility in the work environment.
Ability to react effectively to the needs of patients and their families.
Physical ability to bend, position, transfer and lift patients weighing from 80 pounds to over 300 pounds, without restriction, utilizing clinically recommended lifting devices.
