Work with medically complex patients who challenge and strengthen your clinical skills.

Join a growing provider-led organization serving thousands of seniors throughout Minnesota.

Receive mileage reimbursement, CME support, malpractice coverage, DEA reimbursement, PTO, and comprehensive benefits.

Responsibilities

Provide on-site rounding, primary care, and advanced wound care services.

Help reduce avoidable hospitalizations through proactive intervention and ongoing monitoring.

Communicate with families, caregivers, and interdisciplinary teams regarding care plans and treatment goals.

Complete documentation, medication management, diagnostics, and lab coordination.
