Role: Compensation: Upto $290K salary+ up to 20% of base salary in bonus Competitive sign on bonus Bonus based on: 1. Patient satisfaction rating 2. Living the companys values 3. HEDIS compliance 4. Reducing hospital sick days Primary Care Outpatient Low patient volume - 15-18 patients per day Value Based Care Model Patient population: Seniors with 3-5 complex diseases (Hearth disease, diabetes, hypertension etc.) Benefits: Health, Vision, Dental (including family options) & paid parental leave program Pre-tax or post-tax Roth contributions available.
