Job description
Oak Street Health seeks a Behavioral Health Specialist to provide evidence-based counseling and care coordination for adult Medicare patients in underserved communities. You will complete assessments, develop treatment plans, deliver individual and group therapy, and collaborate closely with primary care providers, nurses, and social workers. The role includes crisis intervention, patient education, and connecting patients with community resources. You’ll document in EHR systems and contribute to a patient-centered, team-based model focused on prevention, equity, and improved health outcomes.
