Job description
We are seeking a Registered Nurse (RN) Home Health Case Manager to oversee patient care in a home setting.
Key Responsibilities Include
Managing a caseload of home health patients, supervising LPN visits, and coordinating with therapy teams.
Conducting assessments and developing personalized care plans aligned with physician orders.
Supporting patients in managing conditions such as CHF, COPD, diabetes, wounds, and medication regimens.
Qualifications And Skills Required
Missouri or Compact RN license
Valid driver's license
Strong organizational, clinical, and communication skills
