Job description
Care2U is a fast-growing healthcare startup delivering on-demand acute and hospital-level care in patients' homes across New York City, Nassau, and Westchester counties. This remote telehealth role requires NY licensure; initial in-person training occurs in Plainview, NY.
You will assess patients via telehealth, coordinate with on-site assistive staff, and manage care plans to reduce ED visits and hospitalizations while ensuring safety and high-quality care.
