Evergreen Healthcare Group is seeking a Regional Central Intake Director to oversee the daily SNF referral intake across regions. This 100% remote, part-time role requires timely responses to referrals, coordination with clinical teams, and accurate entry in PCC CRM.
Ideal candidates have 3–5 years in SNF admissions or related fields, strong communication, and the ability to multitask in a fast-paced environment. Familiarity with portals like EPIC, AIDA, WellSky, Careport, e-fax is a plus.
