Job description
Metropolitan Jewish Health System, Inc. in New York, NY, seeks an experienced RN Case Manager to coordinate care for Elderplan and HomeFirst members enrolled in Medicare/Medicaid plans.
You will assess needs, develop care plans, and work with physicians, caregivers and the care team to improve outcomes. Requirements include NYS RN license, an Associate’s Degree in Nursing (BSN for new grads acceptable), and CCM preferred.
