Job Description
We are seeking a dedicated professional to join our team in a role focused on case management within an acute care setting. This position is critical in coordinating patient care and ensuring compliance with established guidelines and procedures.
Key Responsibilities
Manage a caseload of up to 25 patients, providing effective care coordination and discharge planning.
Ensure compliance with HIPAA guidelines and CMS regulations.
Collaborate with interdisciplinary teams, including social services, to help patients navigate their care.
Handle admissions, denials, and appeals related to patient care.
Conduct needs assessments and manage prior authorizations as required.
Weekend Requirements
This role requires the candidate to work every other weekend as part of their schedule.
Qualifications Minimum of 2 years of acute care experience is required.
Experience with HMOs and IPAs is essential.
Experience with advance directives and the protective services process is preferred.
Knowledge of the Joint Commission standards and core measures is beneficial.
Strong skills in care coordination, patient communication, and case management.
Essential Skills Proficiency in electronic health record systems; experience with Epic is required.
Ability to float between similar units based on departmental needs.
Understanding of pediatric, adult, and geriatric care considerations.
