Job description
Your Health Organization in Blythewood, SC is seeking a Registered Nurse with hospice experience to service patients across the Blythewood area. The Nurse Case Manager will travel to patients’ homes, long-term care facilities, and other sites to evaluate patients and initiate telemedicine visits as part of a comprehensive care plan.
The role emphasizes collaboration with interdisciplinary teams, documentation, and coordination of care, with 12-hour shifts on a full-time basis.
