Job description
BAYADA Home Health Care is seeking an experienced Registered Nurse to fill the role of Transitional Care Manager, supporting Adult and Pediatric Skilled clients across Massachusetts, New Hampshire, Rhode Island, and New York. Travel throughout these states is required.
The Transitional Care Manager will build relationships with hospitals, discharge planners, and referral sources, coordinate safe transitions from facility to home, and monitor outcomes to reduce readmissions.
