nWe are seeking an experienced Pediatric or Family Practice. Due to the expansion of services, Cornell Scott Hill Health Corporation, a leader in health services, is seeking an experienced APRN to join our team! This is an excellent opportunity to work within a nationally recognized Federally Qualified Health Care Center where treating the entire person is at our forefront.
nCornell Scott Hill Health Center is in need of an Advanced Practice Registered Nurses (APRN) to work in our School Based Health Clinics (SBHC). The APRN practices medicine under a collaborative agreement with a departmental physician, with both disciplines practicing within the scope and boundaries of State regulations. Provides reasonable and prudent medical/surgical care, practices appropriate management, and uses sound clinical judgment based on best practice standards and evidenced based medicine. Provides general medical care, patient education and treatment to patients of CS-HHC.
nKey Responsibilities:
nnnCoordinates patient care with colleagues, nursing, front desk outreach, case management staff and other agencies as necessary
n
nnEducates patients on preventive health care and chronic disease management
n
nnProtects clients and employees by adhering to infection-control policies and protocols
n
nnCompletes charting of notes, forms and coding in a timely and efficient manner. Enters appropriate legible notations, documents encounters appropriately
n
nnMaintains confidentiality in regards to patients, staff and program matters at all times
n
n nRequired Qualifications:
nnn3+ years of APRN experience within a healthcare facility
n
nnMSN degree and current CT RN/APRN licensure. CPR certification required
n
nnAdvanced knowledge of EPIC EMRs
n
nnWillingness to work a flexible schedule and to fill in when needed
n
nnKnowledge and management of principles and practices including optimum use of staff and all other available resources.
n
nnAbility to explain treatments and conditions to patients of all ages
n
nnWorking knowledge of health care laws and regulations
n
