Job description
EmpowerMe Wellness is seeking licensed therapy professionals to provide physical, occupational, or speech therapy for older adults within our EmpowerMe 360 program. You’ll help residents live stronger, safer, and more independent lives.
You’ll conduct assessments, create personalized care plans, and deliver evidence-based interventions as part of a collaborative team. You’ll support care coordination with wellness check-ins, caregiver communication, and telehealth options.
