Pay & Benefits Hourly Pay Rate: $63.65

What You'll Do Evaluate patient mobility and develop personalized physical therapy plans
Implement age and developmentally appropriate therapy for pediatric to geriatric patients
Assist patients with transfers and positioning to ensure safety and comfort
Educate patients and families on therapy goals and progress
Document all patient care activities accurately in electronic health records
Collaborate with healthcare team members to coordinate patient treatment
Assess, treat, and evaluate patient progress regularly to meet therapy goals
What You Need Valid Illinois licensed Physical Therapist (PT) license
CPR certification
Prior experience in home health or pediatric therapy preferred
Strong communication and interpersonal skills
Ability to work independently and as part of a healthcare team
