Job description
BAYADA Home Health Care is seeking a Transitional Care Manager (TCM) in Cleveland, Ohio. The RN or LPN will coordinate initiation of home health care services, working with discharge planners, case managers, and families to ensure timely and appropriate care.
The role focuses on building relationships with hospitals, physicians and other health professionals to place patients into BAYADA care, while ensuring compliance with regulatory requirements and internal processes.
