Job description
We are seeking a dedicated Travel Home Health Physical Therapist to provide therapy services in a home health setting.
Role: Physical Therapist (PT) - Home Health
Discipline: Therapy
Responsibilities: Deliver skilled physical therapy services to patients in their homes, assess patient needs, develop and implement treatment plans, and document progress.
Shift: 12-hour shifts, totaling 36 hours per week.
Qualifications & Skills: Relevant licensure as a Physical Therapist, strong clinical skills, ability to work independently, and excellent communication skills.
Key Duties: Provide patient-centered care, evaluate functional status, and collaborate with healthcare teams to support patient recovery and mobility.
