Job Details:
Job Title: Home Health RN
Location: Litchfield, IL (covering Madison, St. Clair, Jersey, Calhoun)
Setting: Home Health
Pay Range:
Schedule: 8am-5pm
Contract Length: 8 weeks
Candidate Type: Traveler or local
Requirements: 1 year of Medicare certified Home Health experience. 1 year of OASIS experience. HomeCare HomeBase EMR experience highly preferred.
Responsibilities:
Provide skilled nursing care in a home health setting.
Assess patient needs and develop individualized care plans.
Monitor patient progress and adjust treatments accordingly.
Collaborate with family members and healthcare team.
Document patient care activities accurately and thoroughly.
