Description
American Traveler is seeking a Physical Therapist for a Home Health position requiring KY state licensure and at least one year of acute care PT experience.
Details
Home health setting covering Massac, Johnson, and Pope counties
Monday through Friday schedule, 8:15am to 5:00pm
One weekend of on-call coverage per month
HomeCare HomeBase charting system preferred but not required
Requirements
KY Physical Therapy State License required if applicable
Minimum of one year of PT experience in an acute care setting required
BLS certification required
Valid driver's license and auto insurance with candidate listed as a driver required
Home care experience preferred
Strong communication skills with patients, families, physicians, and co-workers
Knowledge of therapy best practices and sound clinical judgment
Detail-oriented with strong time management and prioritization skills
Additional Information
Responsibilities include assessment, evaluation, discharge planning, and documentation such as OASIS and plans of care
Utilizes assistive devices and equipment including crutches, gait belts, walkers, and wheelchairs
Focus on home exercise programs, patient and family education, and safety/injury prevention
Mileage reimbursed for travel between patient visits
Open to first-time travelers
