Job description
Role:
Home Health Physical Therapist, providing patient-centered therapy services in patients' homes.
Conduct evaluations, develop treatment plans, and deliver interventions focused on mobility, strength, balance, pain management, and fall prevention.
Perform home safety assessments, recommend environmental modifications, and educate patients and caregivers for long-term success.
Coordinate care with interdisciplinary teams including nursing, occupational therapy, speech therapy, and social work.
Utilize electronic medical records for documentation, care planning, and communication.
Travel between patient homes, performing assessments and follow-up treatments, primarily during daytime hours.
Qualifications & Skills:
Valid Physical Therapist license, driver's license, auto insurance; EPIC experience preferred.
