Job description
Well Care Health seeks an experienced Clinical Resource RN to provide direct nursing services, case management, and care coordination across high-need home health regions. You will deploy to multiple locations, maintain continuity of care, and ensure high-quality patient outcomes while complying with agency standards.
Requirements include an RN license, CPR certification, and a minimum of two years of RN experience, with home health experience preferred.
