Job description
Carle Health is seeking a Home Care RN/Field Case Manager in Illinois to coordinate care plans, perform skilled nursing assessments, and support patient families. The role includes wound care, PICC line care, and collaboration with an interdisciplinary team.
The position requires IL RN licensure, BLS, a valid driver’s license, and at least 1 year of nursing experience. A sign-on bonus of $7,500 is available for external candidates.
