Job description
Kaiser Permanente is seeking a Medical Social Worker (LICSW) to join our team at St. Peter Medical Center. In this role, you will provide psychosocial assessments, short-term counseling, crisis intervention, and discharge planning for diverse patient populations. You will collaborate closely with physicians, nurses, and care coordinators to support holistic care, connect patients and families to community resources, and address social determinants of health.
