Job description
Molina Healthcare is seeking a Care Manager to support LTSS care coordination and work with a multidisciplinary team to deliver integrated member care across the continuum. The role includes in-home assessments, waiver processes, and care plan development to address member needs and goals.
You will promote service integration, assess medical necessity, and participate in ICT meetings while traveling up to approximately 50% of the time. Strong communication and organizational skills are essential.
